Court Forms

  • Pre-Signed Order & Acknowledgement
    The pre-signed (Judge Fink) Order and Acknowledgement is available on the Court Forms page.
  • Updated Forms
    Several court forms have been updated, including guardianship and accounting forms. Please ensure you are using the latest versions.

Accelerated Rulings – ARCP 19 and 15(i)

  • Include the accelerated ruling request in the caption.
  • Explain why an accelerated hearing is needed.
  • A Motion for Accelerated Hearing is still required.
  • Best Practice:
    1. File your petition and obtain a hearing date.
    2. Then file a separate motion for an expedited hearing (do not include this request in your petition; the clerk will not see it).
    3. If possible, obtain opposing counsel’s approval.
  • Tip: Read the rules carefully to avoid delays.

PLAP (Probate Lawyers Assistance Program)

  • Support Needed: PLAP continues to assist 500+ individuals annually.
  • Free Consultations: Attorneys work in shifts each Wednesday to provide six half-hour consultations on various probate, guardianship and/or conservatorship issues.
  • Volunteer Opportunities:
    • You’ll receive an email with the volunteer calendar.
    • No pressure—volunteer when you can.

  • Always check the latest court forms before filing.
  • Follow ARCP rules for accelerated hearings.
